The Client Intake is my way of getting to know who you are as an individual. This form will ask questions about your medical history and about your activities. I use this information to gain a better understanding of you and your needs - and build our session based on that information.

Please fill it out completely so that I can give you the best massage possible.

You will only need to fill this out once.

You can download the form, print it out, and bring the completed form to your first appointment. Don't worry if you forget it, I have extra copies.
